Comparison of Surgical Outcomes in Robot-Assisted Partial Nephrectomy and Laparoscopic Partial Nephrectomy: Analysis of Perioperative and Postoperative Results from a Single Team in Brazil.

Resumo

Background
The comparative research is scarce when it comes to assessing the outcomes of Robot-Assisted Partial Nephrectomy (RAPN) versus Laparoscopic Partial Nephrectomy (LPN).
Objective
Comparing the intra and postoperative results of RAPN and LPN from a single team in Brazil.
Design, setting, and participants
A retrospective analysis included data from 105 patients, 53 undergoing RAPN and 52 undergoing LPN, performed by a team of four experienced surgeons, at two high-volume outsourced centers.
Outcome measurements and statistical analysis
Student's t test was performed, Wilcoxon test, Pearson's chi-square test, Fisher's exact test were used for analysis. Multiple linear regression was employed to determine the relationship between the type of surgery and outcomes with significance established at p < 0.05. Statistical analysis was performed using R software.
Results and limitations
The results indicated that RAPN showed advantages in terms of surgery duration (134.65 vs. 162.57 minutes), tumor size (3.84 vs. 3.40 cm), and length of hospital stay (1.24 vs. 3.05 days; p < 0.001). Additionally, ischemia time was shorter in patients undergoing RAPN compared to those undergoing LPN (13.28 vs. 19.97 minutes; p<0.001). No significant differences were observed in tumor location, ASA score, hypertension, diabetes, age, comorbidities and BMI between groups. Limitations were the retrospective nature of the study and the small sample size.
Conclusion:
Robot-Assisted Partial Nephrectomy (RAPN) yields comparable outcomes to Laparoscopic Partial Nephrectomy (LPN) and may be preferable for complex cases. This research, standing out as one of the retrospective comparisons conducted in Brazil, indicates that RAPN is advantageous, both for novice and experienced laparoscopic surgeons. Further prospective and randomized studies are warranted.
